Job summary

Residential Home Health and Residential Hospice is seeking a compassionate Certified Nursing Assistant to provide hospice care in patients' homes around Evanston, IL. This role offers autonomy, meaningful patient relationships, and opportunities for career growth.

The CNA will deliver hands-on care, assist with ADLs, and support families while documenting care in the EMR. A valid CNA certificate, High School diploma, and driver’s license are required; a sign-on bonus of $5,000 is available.

Qualifications

• Active CNA certificate licensed in the state you work.
• High School Diploma or equivalent.
• Minimum one year healthcare experience; hospice experience preferred.
• Valid driver's license and ability to drive ~20% of workday.

Responsibilities

• Deliver compassionate, patient-centered care in hospice settings.
• Assist with activities of daily living while monitoring changes in condition.
• Provide support to patients and caregivers by reinforcing care plans.
• Document care accurately in the EMR and report findings to the team.
• Work independently while collaborating with interdisciplinary team.
• Travel to patient homes within the assigned territory.

We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Evanston, IL area and is eligible for a $5,000 Sign-on Bonus.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory

CNA Qualifications

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
