Job summary

Superior Hospice is seeking a hospice CNA to provide compassionate, patient-centered care across the 21-County Twin Cities Service Area. You will assist with daily tasks, monitor comfort, and support patients and families in a home setting.

The role emphasizes building meaningful relationships, clear communication with the care team, and adherence to individualized care plans, all while maintaining dignity and independence for patients in their homes.

Qualifications

• Hospice experience preferred, other related care settings valued.
• Warm, dependable, patient-centered.
• Excellent communication and teamwork skills.
• Valid driver's license and reliable transportation required.

Responsibilities

• Assist patients with bathing, dressing, grooming, toileting, and mobility.
• Monitor comfort and report changes to the hospice nurse.
• Provide compassionate companionship and emotional support.
• Help patients remain safe, comfortable, and independent when possible.
• Communicate with nurses and the interdisciplinary team to ensure excellent care.
• Follow individualized care plans with professionalism and compassion.

Compensation: $19-$25/hour DOE + mileage reimbursement paid by wage + $500 Sign-On Bonus

What You'll Do

Who We're Looking For

• Experience in hospice preferred, but home care, long-term care, assisted living, hospital, or oncology experience is also valued.
