Job summary

Superior Hospice is looking for a compassionate Hospice CNA to join our team serving the 21-County Twin Cities area. You will assist patients with daily activities, monitor changes, and collaborate with the interdisciplinary team to ensure comfort and safety in patients' homes.

We offer a $500 Sign-On Bonus, mileage reimbursement, and full benefits for eligible full-time staff, along with meaningful, relationship-focused care that makes a real difference in patients' lives daily.

Qualifications

• Hospice experience preferred, with home care, assisted living, long-term care, hospital, or oncology experience also welcomed.
• Compassionate, dependable, and relationship-focused.
• Strong communication and teamwork skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Assist patients with bathing, dressing, grooming, toileting, and mobility.
• Help patients remain comfortable, safe, and independent.
• Observe changes in condition and communicate with the hospice team.
• Provide encouragement, companionship, and emotional support.
• Follow individualized care plans with professionalism and compassion.
• Work closely with nurses and the interdisciplinary team to provide exceptional care.
