Job summary

Del Corazon Hospice is seeking a dedicated Hospice Certified Nursing Assistant (CNA) to serve patients primarily in Albuquerque, NM, with care in SNFs, assisted livings, and private homes. This full-time position requires compassionate hands-on care and clear communication within an interdisciplinary team.

Qualifications

• Current New Mexico CNA license (required).
• Hospice or home health experience preferred.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to travel daily within service area.
• Strong communication skills and compassionate bedside manner.
• Ability to work independently and as part of a team.
• Must pass background check and drug screening.

Responsibilities

• Provide personal care and emotional support to hospice patients in SNFs and private residences.
• Assist with activities of daily living (bathing, grooming, toileting, transfers, feeding).
• Observe and report changes in patient condition to the clinical team.
• Support families with education and comfort measures.
• Maintain accurate documentation per hospice and regulatory guidelines.
• Work collaboratively with nurses, social workers, chaplains, and the interdisciplinary team.
• Maintain patient dignity, privacy, and comfort at all times.
• Travel throughout the service area daily.
