Job summary

Del Corazon Hospice in Albuquerque, NM is seeking a Hospice Certified Nursing Assistant (CNA) to provide compassionate in-patient and home-based care. You will support patients in SNFs, assisted living, and private homes, delivering personal care and comfort with dignity.

You will travel across the service area daily, maintain accurate documentation, collaborate with nurses and the interdisciplinary team, and help families with education and support.

Qualifications

  • New Mexico CNA license is required.
  • Hospice or home health experience preferred.
  • Valid driver’s license and reliable transportation.
  • Ability to travel daily within the service area.
  • Strong communication and compassionate bedside manner.
  • Ability to work independently and as part of a team.
  • Must pass background check and drug screening.

Responsibilities

  • Provide personal care and emotional support to hospice patients in SNFs and private residences.
  • Assist with activities of daily living (bathing, grooming, toileting, transfers, feeding).
  • Observe and report changes in patient condition to the clinical team.
  • Support families with education and comfort measures.
  • Maintain accurate documentation per hospice and regulatory guidelines.
  • Work collaboratively with nurses, social workers, chaplains, and the interdisciplinary team.
  • Maintain patient dignity, privacy, and comfort at all times.
  • Travel throughout the service area daily.

Position Overview

We are seeking a Hospice Certified Nursing Assistant (CNA) to serve patients primarily in Albuquerque, providing care in Skilled Nursing Facilities (SNFs), Assisted Livings and patients’ homes. This role requires compassionate hands-on care, excellent communication, and a heart for hospice.

Key Responsibilities
  • Provide personal care and emotional support to hospice patients in SNFs and private residences
