Job summary

Concho Hearts Hospice is seeking a compassionate PRN Certified Nursing Assistant (CNA) to care for patients in Mansfield, Texas, and surrounding communities. You will provide hands-on personal care, assist with activities of daily living, and support patients and families with comfort and dignity in their homes and local facilities.

The role requires active Texas CNA certification and strong interpersonal skills.

Qualifications

• Active Texas CNA certification.
• Experience in hospice, home health, assisted living, skilled nursing, or hospital preferred.
• Hospice experience is a plus, but not required.

Responsibilities

• Provide compassionate, hands-on personal care to hospice patients in homes, assisted living, and skilled nursing facilities.
• Assist patients with bathing, dressing, grooming, toileting, and activities of daily living.
• Help patients maintain comfort, dignity, and independence.
• Provide gentle repositioning, mobility assistance, and other personal care within scope of practice.
• Observe and report changes in patient condition to the supervising nurse.
• Offer companionship, encouragement, and emotional support to patients and families.
• Follow individualized plans of care established by the RN.
• Maintain accurate and timely documentation of patient visits.
• Communicate with the interdisciplinary team to ensure patients receive quality care.

What We're Looking For:

• Active Texas CNA certification.
• Previous CNA experience in hospice, home health, assisted living, skilled nursing, or a hospital preferred.
• Hospice experience is a plus, but not required!
• Compassionate, patient, and dependable.
• Strong communication and interpersonal skills.
• Comfortable providing one-on-one personal care.
• Ability to work independently while following the patient's plan of care.
• Strong time-management and organizational skills.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel throughout Mansfield and surrounding communities.
