Job summary

Concho Hearts Hospice is seeking a compassionate Hospice Registered Nurse (RN) for full-time coverage in Aubrey, Frisco, Little Elm, and surrounding communities. You will provide skilled nursing care in patients' homes, perform comprehensive assessments, and develop individualized care plans in a supportive team environment.

Responsibilities include pain and symptom management, coordinating medications and equipment, and educating families about end-of-life care.

Qualifications

• Active Texas RN license (or NLC) is required.
• Experience in hospice, home health, skilled nursing, or acute care is preferred.
• Hospice experience is a plus.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication, organization, and time management.
• Ability to travel throughout Aubrey, Frisco, Little Elm and surrounding communities.
• Current CPR certification is required.
• Valid driver’s license and reliable transportation with current auto insurance.

Responsibilities

• Provide skilled nursing care to hospice patients wherever they call home.
• Complete nursing assessments and regular patient visits.
• Develop, implement and update individualized care plans.
• Provide pain management and comfort-focused care.
• Monitor patient status and coordinate with physicians and the IDT.
• Educate patients and families about medications and end-of-life care.
• Coordinate meds, equipment, and other patient needs.
• Collaborate with LVNs, Hospice Aides, Social Workers, Chaplains and physicians.
• Participate in interdisciplinary team (IDT) meetings.
• Document accurately in the EMR in a timely manner.
• Offer emotional support to patients and families.
• Manage assigned patient care and schedule visits as needed.

What You'll Do:

• Provide skilled nursing care to hospice patients wherever they call home.
• Complete comprehensive nursing assessments and routine patient visits.
• Develop, implement, and update individualized plans of care.
• Provide pain management, symptom control, and comfort-focused care.
• Monitor changes in patient condition and communicate with physicians and the interdisciplinary team.
• Educate patients and families about medications, disease progression, comfort measures, and end-of-life care.
• Coordinate medications, medical equipment, supplies, and other patient needs.
• Collaborate with LVNs, Hospice Aides, Social Workers, Chaplains, and physicians.
• Participate in interdisciplinary team (IDT) meetings.
• Complete accurate and timely documentation in the EMR.
• Provide compassionate emotional support and guidance to patients and families.
• Manage assigned patient care responsibilities and coordinate visits as needed.

What We're Looking For:

• Active Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license.
• Previous nursing experience in hospice, home health, skilled nursing, or acute care preferred.
• Hospice nursing experience is a plus!
• Strong clinical assessment and critical-thinking skills.
• Excellent communication, organization, and time-management skills.
• Ability to work independently while collaborating with the clinical team.
• Compassionate, dependable, and committed to exceptional patient care.
