Job summary

Concho Hearts Hospice is seeking a compassionate PRN Registered Nurse to deliver skilled nursing visits in Kilgore, TX, and surrounding communities. You’ll perform assessments, manage pain and symptoms, and educate families about end-of-life care, medications, and care plans.

You will collaborate with an interdisciplinary team, coordinate care, document in the EMR, and travel locally. A Texas RN license, CPR, and a valid driver’s license are required.

Qualifications

• Active Texas RN license or valid NLC license.
• RN experience in hospice, home health, skilled nursing, or acute care preferred.
• Hospice nursing experience is a plus.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Willingness to travel throughout Kilgore and surrounding communities.

Responsibilities

• Provide skilled nursing visits to hospice patients in homes, assisted living facilities, and SNFs.
• Complete comprehensive nursing assessments and evaluate patient needs.
• Deliver pain management, symptom control, and comfort-focused care.
• Communicate changes in condition to physicians and the interdisciplinary team.
• Educate patients and families about medications, disease progression, and end-of-life care.
• Coordinate admissions, routine visits, or urgent patient needs based on scheduling.
• Document accurately in the EMR and collaborate with care teams.

What You'll Do:

• Provide skilled nursing visits to hospice patients in their homes, assisted living communities, and skilled nursing facilities.
• Complete comprehensive nursing assessments and evaluate patient needs.
• Provide pain management, symptom control, and comfort-focused care.
• Recognize changes in patient condition and communicate with physicians and the interdisciplinary team.
• Educate patients and families about medications, disease progression, comfort measures, and end-of-life care.
• Assist with admissions, routine visits, or urgent patient needs based on scheduling and qualifications.
• Coordinate medications, medical equipment, supplies, and other patient needs.
• Collaborate with RN Case Managers, LVNs, Hospice Aides, Social Workers, Chaplains, and physicians.
• Complete, accurate, and timely documentation in the EMR.
• Provide compassionate emotional support and guidance to patients and families.

What We're Looking For:

• Active Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license.
• Previous nursing experience in hospice, home health, skilled nursing, or acute care preferred.
• Hospice nursing experience is a plus!
• Strong clinical assessment and critical-thinking skills.
• Excellent communication, organization, and time-management skills.
• Ability to work independently while collaborating with the clinical team.
• Compassionate, dependable, and committed to exceptional patient care.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel throughout Kilgore and surrounding communities.
