Job summary

Concho Hearts Hospice is seeking a compassionate Hospice CNA/Hospice Aide to support patients and families in Tyler, TX. This full-time role focuses on comfort, dignity, and personal connection, with the opportunity to join a dedicated interdisciplinary team.

You will provide hands-on care in patients' homes or care facilities, observe changes, document care, and communicate with the hospice team while building trust with patients and their families. Travel locally is required.

Qualifications

Unlike facility settings where assignments can feel rushed, hospice care allows you to focus on comfort, dignity, and personal connection. Our Hospice Aides play a vital role in helping patients feel supported and cared for during one of life's most important journeys.

What You'll Do:

• Provide compassionate personal care to hospice patients in their homes or care facilities.
