Job summary

Concho Hearts Hospice in Waxahachie, TX is seeking a compassionate Hospice RN Case Manager to provide skilled nursing care in patients' homes and local facilities.

You’ll manage caseloads, complete visits, admissions, and recertifications, develop plans of care, collaborate with physicians and the interdisciplinary team, and support patients and families with education and empathy.
