Job summary

Your Health Organization is seeking a Registered Nurse to serve patients across the Aiken, SC area as a Nurse Case Manager. The role requires home and facility visits, care coordination, and telemedicine initiation to support the care team.

The ideal candidate has an active RN license, at least 3 years of clinical experience, and strong communication skills. Travel to patients' homes and facilities is required daily, with a full-time schedule and on-call rotation for nights and weekends.

Qualifications

• Must be a Registered nurse with an active license in good standing.
• Minimum of three (3) years clinical experience preferred.
• Experience in community settings is a plus.
• Proven ability to communicate and collaborate with interdisciplinary teams, patients, and caregivers.

Responsibilities

• Visit patients in their homes and facilities to evaluate and coordinate with the care team.
• Enroll patients in Care Management and initiate a Care Plan of Care when eligible.
• Coordinate telemedicine visits and communication with providers and therapists.
• Reconcile medications and assess needs for DME or adaptive equipment.
• Document Care Management activities and maintain up-to-date care plans.
