Job summary

Residential Home Health and Residential Hospice is seeking a Hospice Certified Nursing Assistant (CNA) to provide in-home care in the Lansing, MI area. You will work with an interdisciplinary team to support comfort, dignity, and quality of life for patients.

Responsibilities include assisting with ADLs, documenting care in the EMR, and traveling to patient homes within the territory. This role offers meaningful patient relationships and career growth with a national leader in home health and

Qualifications

• Active CNA certificate in the state you work.
• High School Diploma or certificate.
• Minimum one year healthcare experience; hospice experience preferred.
• Valid driver’s license and ability to drive ~20% of workday.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Assist with ADLs and observe/report changes to support comfort and quality of life.
• Reinforce care plans, provide emotional support to patients and families.
• Document care provided in EMR accurately.
• Travel to patient homes within assigned territory.
• Work independently, yet never alone.
