Residential Home Health and Hospice in Lansing, MI is hiring a Certified Nursing Assistant to deliver compassionate in-home hospice care. Join an interdisciplinary team focused on comfort, dignity, and quality of life.

As a CNA, you’ll assist with ADLs, observe changes in condition, document care in the EMR, and support patients and caregivers through the hospice journey. Travel to patient homes within territory; valid driver's license required.

Qualifications

• Active CNA certificate in the state you work.
• High school diploma or equivalent.
• Minimum one year healthcare experience; hospice experience preferred.
• Valid driver’s license; ability to travel to patient homes ~20% of day.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting.
• Assist with activities of daily living (ADLs) and observe changes in condition.
• Document care in the EMR and communicate with the interdisciplinary team.
• Support patients and caregivers by reinforcing the care plan and providing emotional support.
• Work independently, but collaborate with the team when needed.
• Travel to patient homes within assigned territory.
