Job summary

Halcyon Home is seeking a dedicated Hospice CNA to support our geriatric hospice patients in North Austin and South Austin, TX. This is a full-time position with competitive pay and strong team support.

You will provide direct patient care under RN supervision, assist with transfers and safety, document care, and promote compassionate communication with patients and families. CNA certification in Texas required; hospice or senior care experience preferred.

Qualifications

• Current Texas CNA certification.
• Experience in hospice or senior care preferred.
• Strong interpersonal and communication skills.

Responsibilities

• Provides direct patient care under RN supervision and according to the Plan of Care.
• Assists with safe transfers and ambulation per Hospice policy.
• Reports changes and documents care rendered to ensure continuity of care.
• Maintains infection control, cleans, and provides comfortable environment.
• Promotes respectful communication with patient/family and staff.
• Assists with nutrition and fluid intake and time management.
• Performs treatments and procedures per Hospice policy.
• Supports Hospice philosophy and policies to ensure quality of care.

Essential Functions:

• Provides direct patient care to patient under direction of the RN and according to the Aide Plan of Care.
• Provides necessary skills to assist the patient with safe transfers and ambulation per the Hospice policy.
• Provides necessary skill to appropriately report changes and document pertinent information and care rendered to
• patient to ensure continuity of care.
• Practices accepted infection control principles. Provide a clean, safe, and comfortable environment.
• Promotes positive, supportive, respectful communication to patient/family and other employees.
• Provides an environment which promotes respect for patient, privacy, and property.
• Provides skills necessary to assist the patient with proper nutrition and adequate fluid intake.
• Contributes to the management and efficient operation of the Hospice and demonstrate effective time management skills.
• Provides skills necessary to perform treatments and procedures according to the Hospice policy.
• Promotes the Hospice philosophy and administrative policies to ensure quality of care.

Requirements

• Current CNA certification in Texas.
• Proven experience in hospice or senior care preferred.
• Strong interpersonal and communication skills.
• A kind and patient demeanor with a commitment to providing dignified care.

Payrate: $25 per visit
