Responsibilities

• Provide direct patient care under RN supervision and Aide Plan of Care.
• Assist with safe transfers and ambulation per Hospice policy.
• Report changes and document care rendered to ensure continuity of care.
• Maintain infection control, and a clean, safe environment.
• Promote respectful communication with patients, families and staff.
• Support privacy, nutrition, and fluid intake for patients.
• Assist in timely management of Hospice operations and time management.
• Perform treatments and procedures per Hospice policy.

Essential Functions:

• Provides direct patient care to patient under direction of the RN and according to the Aide Plan of Care.
• Provides necessary skills to assist the patient with safe transfers and ambulation per the Hospice policy.
• Provides necessary skill to appropriately report changes and document pertinent information and care rendered to
• patient to ensure continuity of care.
• Practices accepted infection control principles. Provide a clean, safe, and comfortable environment.
• Promotes positive, supportive, respectful communication to patient/family and other employees.
• Provides an environment which promotes respect for patient, privacy, and property.
• Provides skills necessary to assist the patient with proper nutrition and adequate fluid intake.
• Contributes to the management and efficient operation of the Hospice and demonstrate effective time management skills.
• Provides skills necessary to perform treatments and procedures according to the Hospice policy.
• Promotes the Hospice philosophy and administrative policies to ensure quality of care.

Requirements

Payrate: $20 per hour / $25 per visit
