Job summary

Partners In Care is seeking a compassionate CNA for Hospice House in Bend, OR. You will deliver personal care per the care plan, assist with mobility and daily activities, and support family members while maintaining patient comfort.

Requirements include a valid CNA license, background check, TB test, and CPR. Hospice/home health experience is preferred; valid Oregon driver license and auto insurance are required.

Qualifications

• Requires valid CNA license.
• Background check, drug screen, and TB test pass required.
• Graduate of an accredited nursing school.
• 1–2 years of recent medical care in an institutional setting; hospice/home health experience preferred.
• Valid Oregon driver’s license and auto insurance.
• CPR certification required.
• Strong communication and interpersonal skills.
• Ability to work independently and with an interdisciplinary team.
• Compassion, empathy, and dedication to patient care.

Responsibilities

• Provide personal care according to the care plan.
• Assess patients before and after treatment and keep the care plan updated.
• Assist with personal care, mobility, and positioning; guide family members.
• Prepare and serve meals and assist with feeding.
• Perform housekeeping to maintain a safe and clean environment.
