Job summary

Tidewell Hospice, a member of Empath Health, is seeking a caring CNA to provide patient care in private homes and care facilities in the South Sarasota and Charlotte County area on a PRN basis. Shifts are 8 hours, with a target of 64 hours per month.

Must hold a Florida CNA license and a valid Florida driver's license if traveling between sites. Responsibilities include following the Hospice Aide Plan of Care, reporting changes to the nurse, maintaining patient confidentiality, and working

Qualifications

• Valid Florida Board of Nursing certification (CNA License) is required.
• Valid Florida State Driver’s License required only if driving to other facilities as part of the job, mileage reimbursement where applicable.

Responsibilities

• Provide personal care and home maintenance tasks per the Hospice Aide Plan of Care.
• Identify and report any unusual conditions or incidents to the supervising nurse.
• Maintain patient/family confidentiality and work within the Hospice Aide scope of practice.
• Support the care team to deliver Full Life Care in patients' homes, LTC facilities, or assisted living facilities.
