Job summary

Elara Caring in Adrian, MI is seeking a Hospice Certified Nurse Assistant (CNA) to provide compassionate, skilled nursing care in home environments. You will work under the RN and collaborate with the care team to support patients and families, documenting care and medication information while promoting safety and quality outcomes.

The role requires a high school diploma or GED and at least 1 year of Hospice Aide or CNA experience, with a state-approved training program and the ability to lift

Qualifications

• 1 year of Hospice Aide or Nursing Assistant experience required.
• State-approved Hospice Aide training program meeting 42 CFR §484.36 and competency evaluation.
• Must be able to lift/push/pull up to 50 pounds and transfer patients.

Responsibilities

• Provide skilled nursing care per Plan of Care and RN guidance.
• Document medications and maintain current profiles.
• Educate patients and caregivers on self-care techniques.
• Collaborate with care team to update the Plan of Care as needed.
