Job summary

Elara Caring in Kalamazoo is seeking a Hospice Certified Nurse Assistant/Hospice Aide to join a dedicated home health team. You will provide skilled nursing care per the Plan of Care, monitor patients, and educate families to promote safety and independence.

The role requires a HS diploma or GED and 1 year of hospice aide or nursing assistant experience, with a valid driver’s license and ability to lift up to 50 lb. Hospice experience preferred.

Qualifications

• High School Diploma or GED equivalent required.
• 1 year of experience as a Hospice Aide or Nursing Assistant is required.
• State-established hospice aide training program per 42 CFR §484.36 and a competency evaluation program.

Responsibilities

• Provide skilled nursing care according to the Plan of Care, physician orders, and RN direction.
• Monitor patient condition and promptly report changes to the RN Case Manager and care team.
• Collaborate with the RN Case Manager to update the Plan of Care as needed.
• Educate patients and caregivers on self-care techniques to support safety and independence.
• Document medication regimens and maintain accurate medication profiles.
• Help prevent avoidable hospitalizations through coordinated patient care.
