Dover Health seeks a compassionate Hospice Certified Nursing Assistant to deliver hands-on personal care and daily living assistance under an RN in patients’ homes across Aurora, Oswego, and Yorkville. You will document care, use medical equipment safely, and support families with empathy.

The role requires CNA and CPR certifications, reliable transportation, and strong communication. Travel and collaboration with a multidisciplinary team are essential.

Formación

• Current Certified Nurse Aide (CNA) certification.
• Current Basic Life Support/CPR certification.
• Preferred hospice or home care experience.
• Current and valid driver's license, reliable transportation.
• Excellent communication and teamwork abilities.
• High attention to detail and accuracy.
• Ability to work with dignity and empathy in end-of-life care.

Responsabilidades

• Fulfilling the Hospice CNA Plan of Care in collaboration with the supervising RN.
• Collaborating with the care team to address patient needs.
• Delivering hands-on personal care and assistance with activities of daily living.
• Documenting care provided per agency policy.
• Using Durable Medical Equipment safely.
• Promoting patient safety and regulatory compliance.
• Participating in professional development and lifelong learning.
• Performing other duties as assigned.
