Job summary

Pallatus Health is seeking a caring Certified Nursing Assistant to provide hands-on hospice care in patients' homes and care facilities, under RN supervision, ensuring comfort and dignity at the end of life. You will perform ADL assistance, monitor conditions, document care, and support families, with a focus on compassionate communication and infection control.

Qualifications

• Must hold a CNA certification in the state of practice.
• High school diploma or equivalent.
• Minimum 1 year of healthcare experience preferred.
• Compassionate and patient demeanor with strong interpersonal skills.
• Ability to lift up to 50 pounds and perform physical care tasks.
• Reliable transportation and valid driver's license for home visits.
• Ability to follow care plans and work independently.
• Current CPR certification preferred.
• Preferred hospice, home health, or long-term care experience.
• Experience with end-of-life care is a plus.

Responsibilities

• Provide personal care including bathing, grooming, and ADL assistance.
• Monitor and report changes in patient condition to the RN.
• Assist with comfort measures including positioning, skin care, and mobility.
• Take and record vital signs as directed by nursing staff.
• Provide emotional support and companionship to patients and families.
• Assist with light housekeeping tasks related to patient care areas.
• Help with meal preparation and feeding assistance when needed.
• Maintain accurate documentation of care provided.
• Respect patient privacy, dignity, and cultural preferences.
• Follow infection control and safety protocols.
• Communicate effectively with interdisciplinary team members.
