Job summary

Seasons Hospice is seeking a compassionate Certified Nursing Assistant to provide patient-centered care in home or facility settings. You will assist with daily living activities, monitor health, and offer emotional support to patients and families during life’s important moments.

Qualifications

• High school diploma or equivalent preferred.
• Completion of an approved CNA program.
• At least 1 year direct care experience preferred.
• Ability to lift up to 75–100 pounds.
• Must pass pre-employment screenings and background checks.

Responsibilities

• Assist with bathing, grooming, dressing, toileting, and personal hygiene.
• Change linens, wash laundry, clean living areas, and maintain a safe environment.
• Measure and report vital signs, intake & output, and observe/report health changes.
• Assist with feeding, hydration, ambulation, and patient positioning.
• Administer basic treatments and support medication adherence as directed by nurses or physicians.
• Prepare meals and assist with nourishment between meals.
• Offer companionship, conversation, and emotional support.
• Accompany patients to medical appointments or errands as needed.
• Document care and communicate with the nursing team.
• Assist with post-mortem care as needed in hospice settings.
