Job summary

Crown & Seasons Hospice invites compassionate CNAs to join our team providing care in patients' homes or facilities. You will support daily living activities, monitor health changes, and offer emotional and physical comfort to patients and families during challenging times.

Responsibilities include bathing, meal assistance, mobility support, vital signs, and documentation. Join us for a meaningful role with flexible scheduling and opportunities for growth, in a patient-centered hospice setting.

Qualifications

• Strong compassion and desire to make a difference in patient care.
• Reliable, with excellent communication with patients, families, and nurses.
• Comfort with end-of-life care and hospice environments.
• Willingness to assist in all aspects of patient care and maintain dignity.

Responsibilities

• Assist with bathing, grooming, dressing, toileting, and personal hygiene.
• Change linens, wash laundry, clean living areas, and maintain a safe environment.
• Measure and report vital signs and health changes.
• Assist with feeding, hydration, ambulation, and positioning.
• Support medication adherence under nurse supervision.
• Prepare meals and assist with nourishment as needed.
• Provide companionship and emotional support to patients and families.
• Accompany patients to appointments or errands as required.
• Document care and communicate with the nursing team.
• Support post-mortem care as needed in hospice settings.
