Job summary

Elara Caring in the United States seeks a Hospice Certified Nurse Assistant/Hospice Aide to join our home care team. You will deliver skilled nursing support per the Plan of Care, assist patients and families, and coordinate with the RN Case Manager to maintain high quality care.

This role requires a HS diploma or GED, 1 year hospice or nursing assistant experience, and ability to lift up to 50 pounds. We offer comprehensive benefits, employer 401(k), tuition reimbursement, and paid time off.

Qualifications

• 1 year of Hospice Aide or Nursing Assistant experience is required.
• State-established Hospice Aide training program with competency evaluation.
• Ability to frequently lift up to 50 pounds and assist patient handling.

Responsibilities

• Provide skilled nursing care according to the Plan of Care and RN direction.
• Monitor patient condition and report changes to the RN Case Manager and care team.
• Collaborate with the RN Case Manager to update the Plan of Care when needed.
• Educate patients and caregivers on self-care to support safety and independence.
• Document medication regimens and maintain up-to-date profiles.
• Help prevent avoidable hospitalizations through proactive care.
• Provide compassionate care with positive communication to patients and families.
