Job summary

Elara Caring is seeking a Certified Nurse Aide to join our home care team. You will provide compassionate, hands-on personal care under a Registered Nurse, assist with daily activities, monitor patients’ vitals, and support mobility in patients’ homes.

You will follow individualized care plans, document accurately, and respond to emergencies while maintaining a safe, clean environment. A CNA/HHA certification and ability to travel between patient homes are required.

Qualifications

• Current State Home Health Aide or Certified Nursing Assistant certification is required.
• Ability to frequently lift up to 50 pounds and assist with transfers and mobility.
• Valid driver's license or reliable transportation to travel between patient homes.

Responsibilities

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient's care plan and monitor changes in condition.
• Maintain a clean, safe home environment and protect patient privacy.
• Remind patients to take medications as directed and document accurately.
• Respond to emergencies and follow infection control guidelines.
