Job summary

Elara Caring in the United States is seeking a Hospice Certified Nurse Assistant/Hospice Aide to join our devoted team delivering compassionate care in patients' homes. You will implement the Plan of Care, monitor conditions, and work with the RN Case Manager to adjust care as needed.

Qualifications

• 1 year of Hospice Aide or Nursing Assistant experience.
• State-established Hospice Aide training program meeting 42 CFR §484.36.
• Ability to lift up to 50 pounds and assist with patient transfers.
• Valid driver's license or reliable transportation to travel between homes.
• Experience in a home health environment preferred.

Responsibilities

• Provide skilled nursing care according to the Plan of Care and physician orders.
• Monitor patient condition and report changes to the RN Case Manager.
• Collaborate with the RN Case Manager to update the Plan of Care as needed.
• Educate patients and caregivers on self-care to support safety and independence.
• Document medication regimens and maintain accurate profiles.
• Help prevent avoidable hospitalizations through coordinated care.
• Provide compassionate care with positive communication with care teams.

Job Description:

As a Hospice C.N.A /H.H.A., you’ll contribute to our success in the following ways:

• Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
• Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
• Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
• Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
• Document medication regimens and maintain accurate, up-to-date medication profiles.
• Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
• Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.

What is Required?

• High School Diploma or GED equivalent required
• 1 year of experience as a Hospice Aide or Nursing Assistant is required
• State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a home health environment is preferred
