Bristol Hospice Modesto is hiring a Certified Home Health Aide (CHHA) to join our Interdisciplinary Group Team. You will work under a Registered Nurse, providing personal care and comfort to patients to meet their daily needs.

Qualifications

• Must be a CNA in good standing and have a California HHA in good standing.
• Prefer 1 year of community health/hospice or medical/surgical experience.
• Must have completed a minimum of 75 hours of classroom and supervised practical training.

Responsibilities

• Perform personal care and attend to patient requests promptly.
• Provide patient and family with positive, ethical communication and confidentiality.
• Ensure safety and use equipment properly (foot stools, side rails, O2).
• Assist with bathing, hygiene, grooming, and changing bed linen.
• Assist in dressing, meals, feeding, and documentation.
• Take and record temperatures, pulse, respiration, and BP as ordered.
• Observe skin and prevent tissue breakdown; report changes to Case Manager.
• Assist with ambulation, transfers, and ROM as directed.
• Assist with self-administered medications under state rules.
• Provide respite for caregiver when on-site and keep living area clean.
