Job summary

Bristol Hospice Modesto is hiring a Certified Home Health Aide (CHHA) to support patients in their homes under RN supervision. The role focuses on compassionate personal care, safety, and comfort in a hospice setting.

The ideal candidate holds a CA CNA and HHA certification, with a preference for 1 year of hospice or community health experience and a valid driver’s license. CPR certification is required.

Qualifications

• Must be a CNA in good standing with CA HHA certification in good standing.
• Preferred 1 year of community health/hospice or medical/surgical experience.
• Minimum 75 hours of classroom and supervised practical training completed.
• Licensed driver with insured vehicle in good working order.
• Willingness to submit to a criminal background check annually.
• Current CPR certification.

Responsibilities

• Perform personal care and attend to patient requests promptly under supervision.
• Use positive communication techniques while maintaining confidentiality.
• Meet safety needs of patient and use equipment safely.
• Provide bathing, grooming, oral care, and changing bed linens as assigned.
• Assist with dressing, meals, and feeding as assigned.
• Record vital signs and report significant changes to Case Manager.
• Observe skin condition and prevent tissue breakdown; support mobility and ROM.
• Assist with medications reminders and self-administration as allowed by state law.
• Support caregiver in the home and maintain a clean living area.
