Job summary

Bristol Hospice Modesto, CA is seeking an experienced Certified Home Health Aide (CHHA) to join our compassionate team. You will work under the supervision of a Registered Nurse and provide personal care and comfort to patients, supporting their families.

We value integrity, respect, and excellence. This PRN hourly position offers weekend hours Saturday-Sunday 8:00am-5:00pm with a competitive pay range of $23.00-$25.00 per hour.

Qualifications

• Must be a CNA in good standing and California HHA certification.
• Preferred 1 year experience in community health/hospice or medical/surgical settings.
• Minimum of 75 hours classroom and supervised practical training.
• Licensed driver with insured vehicle and good working order.
• Criminal background check required before hire and annually thereafter.
• Current CPR certification.

Responsibilities

• Perform personal care tasks and promptly respond to patient requests.
• Provide positive communication to patients and families with confidentiality.
• Ensure safety and proper use of equipment and supplies.
• Assist with dressing, meals, and daily living activities.
• Record vitals and report changes to the Case Manager.
• Assist with mobility, transfers, and range of motion.
• Provide medication reminders as allowed by state regulations.
• Maintain patient living area in a clean, orderly condition.
