Job summary

You will follow the plan of care with the nursing team, document changes, assist with meals and mobility, and help patients maintain independence while ensuring safety and dignity.

Qualifications

• High school diploma or equivalent.
• Current state CNA certification/registration.
• CPR certification.
• Valid drivers license and reliable transportation.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to the performance of personal and supportive care.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents changes in patient status and reports to the nurse.
• Follows written instructions for personal care prepared by a registered nurse.
• Assists patients with prescribed exercises as instructed by a nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.
