Job summary

Transitions Hospice - Indiana in Indianapolis is seeking a compassionate CNA to join our home-based care team. This PRN role serves patients in Indianapolis and surrounding territories, delivering personal care with dignity and support in the patients' homes.

You will work with an interdisciplinary team, monitor vital signs, document care, and communicate with patients and families. Competitive pay and travel locally with company-provided equipment are offered.

Qualifications

• Active CNA license or certification in the state of employment.
• Current CPR certification required.
• One year or more hospice/home health experience preferred.
• Comfortable working with terminally ill patients and families.
• Valid driver's license and willingness to travel locally.

Responsibilities

Pay Range: $18.00 - $23.00 an hour

What You'll Do

As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team - not just a support role, but a trusted presence for patients and families. Your responsibilities include:

What You Bring
