Job summary

Transitions Hospice in Indianapolis is seeking a compassionate CNA to join our interdisciplinary care team for in-home hospice visits in Indianapolis and surrounding areas. This PRN as-needed role focuses on patient comfort and dignity, with strong emphasis on compassionate presence for patients and families.

Responsibilities include monitoring vitals, documenting observations, and communicating with the care team. A valid CNA license and CPR are required, with willingness to travel locally.

Qualifications

• Active CNA license or certification in the state of employment.
• Current CPR certification.
• Hospice, home health, or related experience preferred.
• Comfort with terminally ill patients and families.
• Calm, dependable presence under emotionally demanding circumstances.
• Valid driver's license and willingness to travel locally for patient visits.

Responsibilities

• Provide personal care with dignity, comfort, and respect at the center of everything
• Attend to patient needs and requests promptly and compassionately
• Monitor and record vital signs (temperature, pulse, respiration, blood pressure) as directed
• Observe patients and reporting changes to the care team accurately and in a timely manner
• Communicate with patients and families in a positive, ethical, and confidential manner
• Maintain thorough documentation in line with organizational standards
• Uphold the hospice philosophy with professionalism and genuine compassion
