Job summary

Transitions Hospice - Illinois is seeking a compassionate CNA to provide in-home patient care across Streator, Ottawa, Morris and surrounding areas. You will work Monday through Friday, with 8-hour shifts, delivering dignified personal care and monitoring patient needs in a home setting.

Qualifications

• Active CNA license or certification in Illinois
• CPR certification current
• Hospice, home health, or related experience preferred (1+ year)
• Comfort with terminally ill patients and families
• Calm, dependable presence under emotionally demanding circumstances
• Strong attention to detail in observation and documentation
• Willingness to travel locally for patient visits

Responsibilities

• Provide personal care with dignity, comfort, and respect at the center of everything
• Attend to patient needs and requests promptly and compassionately
• Monitor and record vital signs as directed
• Observe patients and report changes accurately and timely to the care team
• Communicate with patients and families in a positive, ethical, and confidential manner

Salary Range: $19.00 - $23.00 an hour

What You'll Do

As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families. Your responsibilities include:

• Providing personal care with dignity, comfort, and respect at the center of everything
• Attending to patient needs and requests promptly and compassionately
• Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed
• Observing patients and reporting changes to the care team accurately and in a timely manner

• Upholding the hospice philosophy with professionalism and genuine compassion

Requirements

What You Bring

• Active CNA License or Certification in the state of employment
• Possess and maintains current CPR certification
• One (1) + year Hospice, home health or related organization experience preferred
• Comfort and sensitivity working with terminally ill patients and their families
• A calm, dependable presence under emotionally demanding circumstances
• Strong attention to detail in observation and documentation
• A valid driver's license and willingness to travel locally for patient visits
