Transitions Hospice - Illinois is seeking a compassionate Companion Aide CNA for the overnight shift. You will provide bedside presence, monitor patient safety, and assist with personal care to support families through the night.

The role requires a current CNA certification and CPR certification, with preferred hospice or home health experience. Night shifts and rotating weekends offer a meaningful, patient-centered schedule.

Qualifications

• Current CNA certification meeting state training requirements.

• 1+ year of hospice, home health, or related healthcare experience preferred.
• Overnight or night-shift care experience preferred.
• Ability to work independently and stay alert overnight.
• Strong observational skills to recognize changes in patient condition.
• Genuine compassion for patients and families navigating end-of-life care.

Responsibilities

• Provide continuous bedside presence and patient supervision overnight.
• Monitor patient condition, behavior, and environment; report changes to the RN on-call.
• Prevent falls, elopement, or self-harm through attentive observation and CNA-led interventions.
• Assist with personal care including repositioning, toileting, bathing, hygiene, and dressing.
• Document observations and vital signs when ordered and as competency allows.
• Support families with overnight respite and compassionate communication.
• Operate equipment safely and assist with ambulation per plan; participate in PI activities.

Schedule: 8pm - 8am 3 days a week with rotating weekends

Salary Range: $20.00 - $23.00 an hour

What You'll Do

As a Companion Aide CNA on the night shift, you'll be a steady, compassionate presence at the bedside — monitoring patient safety, providing personal care, and giving families the peace of mind that someone is watching over their loved one through the night. Your shift includes:

Overnight Patient Care & Safety

• Providing continuous bedside presence and patient supervision throughout the overnight shift to ensure safety and comfort


• Monitoring patient condition, behavior, and environment — and promptly reporting any significant changes to the on-call nurse


• Preventing falls, elopement, or self-harm through attentive observation and appropriate intervention within CNA scope of practice


• Responding promptly to patient requests and ensuring a calm, restful overnight environment


• Assisting with personal care including repositioning, toileting, bathing, hygiene, and dressing as assigned


• Providing proper skin care and observation to prevent tissue breakdown during prolonged periods of rest


• Supporting normal range of motion and repositioning to maintain patient comfort and skin integrity

Observation & Documentation

• Observing and documenting patient condition, sleep patterns, behaviors, and notable changes throughout the shift


• Taking and recording vital signs — temperature, pulse, respirations, blood pressure, and weights — when ordered and within demonstrated competency


• Reporting significant changes in patient condition to the RN or on-call supervisor promptly and without delay


• Adhering to the organization's documentation standards and professional conduct expectations

Family Support & Communication

• Providing families and caregivers with reassurance and overnight respite support


• Maintaining respectful, ethical communication with patients and families, including strict confidentiality


• Demonstrating comfort and compassion in providing specialized care to terminally ill patients and their families

Safety & Equipment

• Using patient equipment safely and appropriately — including foot stools, side rails, and oxygen


• Assisting with ambulation, exercise, and range of motion as directed by the hospice nurse or therapist


• Participating in quality assessment and performance improvement activities
