Job summary

Elara Caring in Alabama is seeking a dedicated Hospice Aide (CNA) to join our home health team. You will deliver compassionate, patient-centered care per the Plan of Care, monitor conditions, document medications, and collaborate with the RN Case Manager to update care plans.

Qualifications

• High School Diploma or GED equivalent
• 1 year of Hospice Aide/Nursing Assistant experience
• State training program meeting CFR 484.36 requirements
• Ability to lift up to 50 pounds
• Valid driver's license or reliable transportation between patient homes
• Home health experience preferred

Responsibilities

• Provide skilled nursing care according to the Plan of Care and RN guidance.
• Monitor patient condition and report changes to the RN Case Manager.
• Collaborate with RN Case Manager to update the Plan of Care as needed.
• Educate patients and caregivers on self-care techniques.
• Document medications and maintain up-to-date medication profiles.
• Help prevent avoidable hospitalizations through coordinated care.
• Maintain compassionate communication with patients, families, physicians, and care team.
