Job summary

Elara Caring is hiring a Hospice Aide (CNA) to join our team in Illinois. You will provide skilled nursing care under the Plan of Care, monitor patient conditions, and coordinate with the RN Case Manager to update the Plan of Care as needed.

The role emphasizes patient education, medication documentation, and preventing hospitalizations. Requirements include a High School Diploma or GED, 1 year of Hospice Aide/Nursing Assistant experience, and a recognized training program meeting regulatory

Qualifications

• High School Diploma or GED equivalENT required
• 1 year of experience as Hospice Aide or Nursing Assistant required
• Training program meeting 42 CFR §484.36 and competency evaluation required

Responsibilities

• Provide skilled nursing care according to Plan of Care
• Monitor patient condition and report changes to RN Case Manager
• Collaborate with RN Case Manager to update Plan of Care
• Educate patients and caregivers on self-care techniques
• Document medication regimens and maintain medication profiles
• Help prevent avoidable hospitalizations through coordinated care
• Provide compassionate care with clear communication with care team
