Job summary

Serenity Hospice PA is seeking a compassionate Certified Nursing Assistant to join the Wyoming Valley team. You will provide in-home and care-facility support, assisting with daily living activities and comfort-focused care, under the supervision of a nurse.

The ideal candidate holds PA CNA certification, is reliable, and able to travel within the Wyoming Valley service area. Hospice experience is preferred, and a valid driver’s license is required.

Qualifications

• Current Pennsylvania Certified Nursing Assistant certification is required.
• Hospice, home health, long-term care, or personal-care experience preferred.

Responsibilities

• Provide personal care to hospice patients in their homes and care facilities.
• Assist with bathing, grooming, dressing, toileting and daily living activities.
• Provide repositioning, mobility assistance, and comfort-focused care.
• Observe and report changes in patient condition to the nurse.
• Follow each patient’s care plan and maintain visit documentation.
• Offer compassionate emotional support to patients and families.
• Communicate effectively with nurses and interdisciplinary team members.
• Maintain patient dignity, privacy, safety, and comfort.

Responsibilities
