As a Hospice CNA, you will work closely with our interdisciplinary team to provide high-quality personal care that promotes dignity, comfort, and quality of life for patients receiving hospice services in patients' homes and care facilities.
Provide personal care including bathing, grooming, dressing, toileting, and mobility assistance.
Observe and report changes in patient condition to the supervising nurse.
Document patient care accurately and timely.
Promote patient comfort, dignity, and independence.
Build compassionate relationships with patients and their families.
Follow all hospice policies, procedures, and infection control standards.
Current Pennsylvania Certified Nursing Assistant (CNA) certification.
Valid driver's license and reliable transportation.
Ability to travel throughout the assigned service area.
Excellent communication and interpersonal skills.
Hospice, home health, or long-term care experience is preferred but not required.
