Job description

Per Diem Hospice CNA – Downingtown, PA

The Hospice CNA provides personal care and light housekeeping services to terminally ill patients in the home, nursing facility, inpatient units or other environments in accordance with an established plan of care under the supervision of a Registered Nurse (RN). The CNA actively participates as a member of the Hospice Interdisciplinary Team, observes and reports patient conditions, documents care performed, and ensures timely documentation of services based on the patient’s care plan.

Responsibilities
  • Provide personal care and light housekeeping to terminally ill patients.
  • Assist patients in accordance with an established care plan under RN supervision.
  • Actively participate as a member of the Hospice Interdisciplinary Team.
  • Observe, report, and document patient observations and care performed.
  • Ensure timely documentation of services based on the patient’s care plan.
Basic Qualifications
  • High school diploma or GED required.
  • Completion of a training program outlined by Medicare Hospice Conditions of Participation 418.76 and approved by the Pennsylvania Board of Nursing.
  • Passing score on a written competency test in accordance with 42 C.F.R. § 484.36 or 418.76(c) prior to working independently with a patient.
  • Valid driver’s license and reliable transportation that is insured per PA requirements.

Experience
  • Minimum age requirement of eighteen (18).
  • One (1) year of direct patient care experience in an institutional or home care setting; community health/hospice experience preferred.

  • Understanding of hospice philosophy and the needs of terminally ill patients.
