Job summary

CareRing Health is seeking a Certified Nursing Assistant for hospice care covering Bucks and Philadelphia Counties. The role involves nursing procedures and patient care under RN guidance and adherence to the CNA care plan.

The CNA will report changes in patient conditions to the RN Case Manager and perform personal cares, documentation, and essential household tasks while ensuring compliance with hospice policies and CE requirements.

Qualifications

• High School Graduate preferred.
• Must have current, Valid Pennsylvania Nursing Aid license.
• Previous Hospice/Palliative experience.

Responsibilities

• Reports changes in the client’s condition and/or needs to the RN Case Manager
• Provides personal cares, including bathing, shampooing, skin and nail care, oral hygiene, shaving & dressing.
• Performs Household task essential to health care, including meal preparation and feeding, laundry & light housekeeping.
• Accurately documents provided cares and submits the documents and pertinent observations following Hospice policy & Procedures
• Attends and completes in-service to ensure compliance with the state requirement of 12 continuous education hours per year.
• Communicates effectively with all members of the interdisciplinary team
• Other duties as assigned, monitoring patient vital signs, including body temperature, heart rate, respiration rate and blood pressure.
