Job summary

Immediate Home Care is seeking a Certified Nursing Assistant for a Hospice role serving Bucks and Philadelphia Counties. You will administer patient care under the RN's direction and according to the CNA plan of care, while documenting observations and ensuring compliance with Hospice policies.

The position requires a current PA Nursing Aide license, High School Diploma, and prior Hospice/Palliative experience. Expect collaboration with an interdisciplinary team and ongoing education hours.

Qualifications

• High School Graduate preferred.
• Must have current, Valid Pennsylvania Nursing Aide license.
• Previous Hospice/Palliative experience.

Responsibilities

• Administer patient care under the direction of RN and per the CNA care plan.
• Report changes in client condition to the RN Case Manager.
• Provide personal care (bathing, hair, skin, nails, oral hygiene, shaving, dressing).
• Perform household tasks essential to health care (meal prep, laundry, light housekeeping).
• Document care accurately and submit observations per Hospice policies.

Job description

Overview

The Certified Nursing Assistant performs nursing procedures and patient care tasks as outlined by a patient’s plan of care. This is a Hospice Role, and will cover Bucks & Philadelphia Counties.

Responsibilities

• Administer patient care under the direction of RN and as instructed by the CNA care plan.
• Reports changes in the client’s condition and/or needs to the RN Case Manager
• Provides personal cares, including but not limited to bathing, shampooing, skin and nail care, oral hygiene, shaving & dressing.
• Performs Household task to essential to health care, including but not limited to meal preparation and feeding, laundry & light housekeeping.
• Accurately documents provided cares and submits the documents and pertinent observations following Hospice policy & Procedures
• Attends and/or completed in-service to ensure compliance with the state requirement of 12 continuous education hours per year.
• Communicates effectively with all members of the interdisciplinary team
• Other duties as assigned, monitoring patient vital signs, including body temperature, heart rate, respiration rate and blood pressure.
