Job summary

Immediate-Homecare- seeks a Certified Nursing Assistant to provide direct patient care in a hospice setting serving Bucks & Philadelphia Counties in Pennsylvania. Under RN supervision, you will follow the CNA care plan and document observations as part of the care team.

The role requires current PA CNA licensure, a High School diploma, and hospice or palliative experience. You will perform personal care, manage basic household tasks, and monitor vital signs, contributing to compassionate,

Qualifications

• High School Graduate preferred.
• Current, valid Pennsylvania Nursing Assistant license.
• Previous Hospice/Palliative experience.

Responsibilities

• Administer patient care under the direction of RN and as instructed by the CNA care plan.
• Reports changes in the client’s condition to the RN Case Manager.
• Provides personal cares, including bathing, shampooing, skin and nail care, oral hygiene, shaving & dressing.
• Performs household tasks essential to health care, including meal preparation and feeding, laundry & light housekeeping.
• Accurately documents provided cares and submits the documents and pertinent observations following Hospice policy & Procedures.
• Attends in-service to ensure compliance with the state requirement of 12 continuous education hours per year.
• Communicates effectively with all members of the interdisciplinary team.
• Other duties as assigned, monitoring patient vital signs, including body temperature, heart rate, respiration rate and blood pressure.
