Qualifications

• Must have a valid driver’s license and reliable transportation, insured per PA requirements.

• Understanding of hospice philosophy and needs of terminally ill patients.

Responsibilities

• Provide personal care and light housekeeping to terminally ill patients.
• Assist patients according to established care plan under RN supervision.
• Participate in Hospice Interdisciplinary Team.
• Observe, report, and document patient observations and care.
• Ensure timely documentation based on patient’s care plan.
