Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• BSN degree preferred or at least one year of nursing experience in the last three years.
• Experience in hospice/home care and two years of management or supervisory experience.
• Complies with accepted professional standards and practice.
• HCHB and PointCare experience preferred.
• Excellent observation, nursing judgment and communication skills; understands hospice philosophy.
• Understands pain/symptom management.
• Must be a licensed driver with insured vehicle; CPR certification required.
• Demonstrates strong written and verbal communication skills.

Responsibilities

• Receives case referrals, assigns hospice personnel, and consults Medical Director on eligibility.
• Reviews and evaluates cases to promote effective home care and quality service.
• Reviews medical diagnoses, prognosis, medications and clinical course.
• Performs medical records management and EMR workflow.
• Assists clinicians in setting immediate and long-term goals and care plans.
• Attends case conferences to coordinate care with an interdisciplinary team.
• Conducts quarterly record reviews and communicates findings to leadership.
• Assists in screening and orienting new employees; supports onboarding.
• Participates in policy development, education planning and program evaluation.
• Complies with professional standards and promotes customer service orientation.
• Pursues quality assessment and performance improvement activities.
