Job summary

Affinity Care of Sarasota seeks a Clinical Manager (RN) to coordinate and manage patient care in a hospice/home care setting. The role involves supervising clinical staff, reviewing cases, and guiding clinicians to deliver high-quality services.

Responsibilities include care coordination, case conferences, and development of plans of care, with emphasis on adherence to professional standards and ongoing staff development. Strong leadership and communication are essential.

Qualifications

• Active RN license in the state of Florida.
• BSN preferred; 1 year of nursing experience within the last 3 years.
• Hospice/home care experience with 2+ years of management or supervision.
• Complies with accepted professional standards; CPR certified.
• HCHB and Pointcare experience preferred.

Responsibilities

• Receives case referrals and assigns hospice personnel as needed; consults Medical Director on eligibility.
• Reviews and evaluates cases via home visits, conferences, records, and clinician services to improve care.
• Reviews patient diagnoses, prognosis, medications, procedures, and clinical course.
• Manages medical records and workflow in the EMR system.
• Assists clinicians in planning immediate and long-term goals and care plans.
• Attends case conferences to coordinate care and interdisciplinary involvement.
• Conducts quarterly record reviews and communicates findings to leadership and staff.
• Assists in screening and interviewing new employees; supports orientation.
• Supports education programs and policy development; contributes to organizational goals.
• Assists in development and evaluation of policies and procedures.
• Promotes adherence to professional standards and public health initiatives.
• Contributes to quality assessment and performance improvement activities.
• Performs other duties as delegated by Clinical Director/Director of Patient Care Services.
