Job summary

Affinity Hospice Management LLC is seeking a Clinical Manager to coordinate patient care and supervise clinical staff. The role ensures services are delivered effectively within hospice/home care and involves interdisciplinary collaboration.

Responsibilities include case assignment, case reviews, medical record management in the EMR, and development of care plans. A BSN is preferred along with prior hospice experience and supervisory duties, plus CPR certification and a valid driver’s license.

Qualifications

• Currently licensed RN in the state.
• BSN preferred or minimum 1 year nursing experience within last 3 years.
• Hospice/home care experience; 2 years management or supervisory experience.
• CPR certification current.
• Must have valid driver’s license and insured vehicle.

Responsibilities

• Receive case referrals and assign hospice personnel as needed.
• Review and evaluate cases through home visits, conferences, and record reviews to guide clinicians.
• Review patient diagnoses, prognosis, medications, and clinical course.
• Maintain EMR medical records and workflow.
• Assist in establishing immediate and long-term goals and plan of care.
• Attend case conference meetings to coordinate care.
• Participate in quality assessment and performance improvement teams.
• Assist in hiring and orientation of new employees.
• Contribute to policies, procedures, and organizational goals.
