Job summary

Affinity Health Management is seeking a Clinical Manager to coordinate patient care and supervise clinical staff within a hospice/home care setting. The role emphasizes ensuring appropriate service delivery and collaborating with the Medical Director on eligibility and care planning.

The ideal candidate holds an active RN license, preferably a BSN, with prior hospice or home care and supervisory experience. Strong assessment, communication, and CPR credentials are essential.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• BSN preferred or minimum of one year experience as a professional nurse within the last three years.
• Previous experience in hospice/home care setting and with supervisory experience.
• Complies with accepted professional standards and practice.
• HCHB and PointCare experience preferred
• Has excellent observation, good nursing judgment and communication skills.
• Understands hospice philosophy and issues of death/dying.
• Must be a licensed driver with insured vehicle in good working order.
• Maintains CPR certification.
• Demonstrates excellent observation, verbal and written communication skills.

Responsibilities

• Receives case referrals and assigns hospice personnel to cases; consults Medical Director on eligibility for services.
• Reviews and evaluates each case to promote effective performance and quality home care services.
• Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course.
• Performs medical records management and workflow in EMR system.
• Assists clinicians in establishing goals, priorities and plans of care.
• Attends case conference meetings to coordinate care and interdisciplinary collaboration.
• Conducts quarterly record reviews and shares findings with Clinical Director and staff.
• Assists in screening and interviewing new employees and orienting them.
• Supports planning and evaluation of inservice and continuing education programs; helps revise policies.
• Complies with professional standards and practices.
• Assesses community health needs and contributes to plans/programs.
• Participates in quality assessment and performance improvement activities.
• Performs other duties as delegated by Clinical Director.
