All Care VNA, Hospice & Private Home Care Services in Lynn, MA seeks a Hospice Clinical Manager to provide clinical and operational leadership for the hospice care team, ensuring high-quality, patient-centered care across daily operations. The role oversees staff, regulatory compliance, and quality improvement in a collaborative environment.

This full-time position follows a Monday–Friday schedule with four days in the Lynn office and one day remote, offering growth opportunities, tuition

Qualifications

• Active Massachusetts RN license required; BSN preferred.
• Minimum two years hospice or home health experience, with supervisory exposure.
• Strong knowledge of hospice regulations and Medicare Conditions of Participation.
• Excellent verbal and written communication; leadership and problem-solving skills.
• Proficiency with EMR/EHR systems and MS Office.

Responsibilities

• Provide day-to-day leadership of hospice clinical operations and ensure quality patient care.
• Supervise, mentor, and coach clinical staff; conduct evaluations and development.
• Monitor staffing, caseload, and productivity to maintain quality and efficiency.
• Coordinate interdisciplinary care plans and ensure updated plans of care.
• Ensure regulatory compliance and participate in quality improvement initiatives.
• Lead staff education, policy development, and training activities.
• Promote a collaborative culture and strong customer service.
