Job summary

All Care seeks an RN for the VNA Clinical Manager role in the Boston territory, overseeing a clinical team delivering in-home patient care. The role manages operations, staff performance, and regulatory compliance to ensure high-quality services.

The position follows a Monday–Friday schedule with four days in the Lynn office and one day remote, and requires expertise in home health and EMR/EHR systems to coordinate care and improve outcomes.

Qualifications

• Active Massachusetts RN license required.
• MS in Nursing preferred.
• Minimum 1–2 years supervisory experience.
• At least 3 years in home health or related clinical setting.
• Strong knowledge of home health regulations and reimbursement guidelines.
• Proficiency in EMR/EHR systems and Microsoft Office suite.

Responsibilities

• Supervise and support a team of nurses and therapists.
• Review, approve, and oversee individualized patient care plans.
• Ensure compliance with federal, state, and accreditation standards.
• Monitor clinical documentation for accuracy and timeliness.
• Coordinate care with physicians and interdisciplinary teams.
• Track patient outcomes and implement quality improvement initiatives.
• Assist with staff hiring, onboarding, training, and evaluations.
• Manage scheduling and staffing to ensure patient coverage.
• Address patient concerns and clinical issues promptly.
• Attend field visits to patient homes as needed.
