VitalCaring is seeking a Clinical Care Manager (RN) – Hospice to guide patients and families with compassion, lead the plan of care, and coordinate a holistic hospice program across the care team.

This full-time, salaried role emphasizes clinical excellence, leadership, and collaboration with physicians, social workers, chaplains, aides, and caregivers to deliver quality end-of-life care.

Formación

• Active RN license in state of practice.
• 2+ years of leadership, supervisory, or care coordination experience.
• Previous Hospice or Home Health experience required, with strong understanding of end-of-life care.
• Excellent communication skills for patients, families, and teams.
• CPR certification current and valid.

Responsabilidades

• Coordinate hospice patient care across the interdisciplinary team.
• Serve as central point of communication among physicians, caregivers, patients, and internal teams.
• Manage physician orders, entry, approval, and alignment with the plan of care.
• Partner with Intake to support timely admissions and follow up on referrals.
• Lead development and updates of individualized hospice plans of care.
• Support patients and families through education, symptom management, and end-of-life planning.
• Review clinical reports and take appropriate action.
• Participate in IDG meetings and care planning.
